VISITATION B.V.M. CYO SPORTS PARENT FEEDBACK FORM

Dear Parents,

The Visitation CYO program is always looking for ways to improve. One critical area is to receive feedback about your child's sports program experience. At the end of each season we ask all parents to take a few moments to reflect on how the sport your child participated in was received by both parent and child. We look for constructive comments about the sport, coaching, game format, etc. In completing this form please keep the following points in mind:

The completed form is reviewed by the CYO Priest Moderator and Athletic Director ONLY.
Coaches in our program are volunteers who sacrifice a lot of their own time for practices and games, they also undergo training required by the Archdiocese in coaching and working with young children. Their dedication is a great service to the parish.
When providing feedback please take the entire season into perspective, don't react to a single incident, it is important to look at the entire experience.
Finally, upon review of this form, you may be asked to sit with the CYO Priest Moderator and/or Athletic Director to discuss any specific issues that they feel require further review.
