The Visitation CYO is evaluating the possibility of offering Pee Wee Flag Football as a sport this coming Fall.  It is a co-ed/non-contact program.  We are providing this form as a way to solicit responses from parents of children entering grades 1 through 4 in the Fall of 2008.  Based on the feedback received from this survey, the CYO Adult Advisory Board will make a final determination on whether there is enough interest to warrant offering this sport in the fall.

Please remember, as with all CYO programs, parent participation is necessary for the success of this proposed program.  Plans for this program are only in the investigative stage.  We depend on your feedback/support in order to determine the feasibility of instituting this program.
